 The Service Award Form is the form councils should use for submitting their program activities to the State Council.  The form actually encompasses the last two pages of this file.  The front end is the instructions that go with the form.
 
Forms that are suggested for councils to use in planning their activities:
 
 The Council Program chart is designed to allow councils to plan their activities by category (Council, Community, Church, Family and Youth).
 
 The Program Activity Planner helps councils plan and track their activities by month.

 The State KofC Scholarship form The Vermont State Council of the Knights of Columbus has recently announced the 38th Annual Scholastic Achievement Awards Program for 2008 and is seeking to
contact all eligible applicants.
